International Student Forms

  • Address Update - Anytime you move, you are required by law to inform us within 10 days of moving
  • Request Express Mail from BU - If you are unable to pick up your document(s) (I-20, DS-2019, etc.) from campus and need to receive them by mail, you can request an express mail shipment through eShipGlobal. Please follow the steps below to request the shipment:
    • Go to https://study.eshipglobal.com
    • Click on "Student/Scholar Sign Up”
    • Register for an account by completing the online form
    • Once you have an account, please log in (if necessary) and click “Receive Package From University”
    • Select “Bellevue University” as the university and “International Programs” as the department.
    • Submit the requested information, including the mailing address for the document
    • Select your express mail shipment option, make the payment, and submit your request
    • Once you’ve submitted your request, International Programs will be notified to ship your document(s)! Please allow a few days processing for the shipment

The following forms will be completed through DocuSign. Please contact International Student Services at [email protected] to request these forms.

  • F-1 Transfer-In Form - Request this form if you are a new student and are currently on F-1 status in the US and would like to transfer to Bellevue University.
  • I-20 Extension Request Form - If your I-20 is expiring soon and you will not be completing your degree before the expiration date, you must have your I-20 extended before your current I-20 expires. Request this form from your immigration advisor.
  • F-1 Reduced Course Load Form - Use this form if you would like to request a reduced course load (RCL) for a given semester. An RCL is a permission from an ISS DSO to enroll below the full-time requirements for a given semester. Federal regulations provide limited circumstances when an F-1 student may be authorized for an RCL. These include medical reasons, academic reasons, and completion of studies during the final term. Request this form from your immigration advisor.
  • F-1 Transfer-Out Form - Use this form if you are transferring to another US School and would like your SEVIS record released to the new school to maintain your F-1 status. Request this form from your immigration advisor.
  • Authorized Early Withdrawal - Use this form to if you plan to withdraw from your studies or leave the US (either temporarily or long-term) before completing your program. You must meet with an ISS immigration advisor to obtain authorization for the withdrawal before you drop any courses. For F-1 students only: Processing your request with your immigration advisor ensures you will have 15 days to leave the U.S. Withdrawing from the university and dropping your courses without first receiving authorization will result in a loss of legal F-1/J-1 status, and does not allow for a 15-day grace period (for F-1 students only). If you are already outside the U.S. and decide not to return to resume your degree/program, you must still complete and be authorized for a withdrawal in order for your immigration record to remain in good standing.
  • NEW I-20/DS-2019 Request - Use this form to request any of the following:
    • Adding a Dependent
    • Duplicate I-20/DS-2019 - If you have lost your I-20/DS-2019, or if it gets damaged or stolen, you must report this to us and request a new I-20/DS-2019 for official use
    • Updating your source of funding on the I-20/DS-2019
    • Updating your change of program (major, degree level, etc.) or biographical data on the I-20/DS-2019
    • A change of status to F-1/J-1 I-20/DS-2019
  • Document Copies Request - If you are requesting copies of old I-20s/DS-2019 or other documents, please complete and submit this form. *There is no guarantee that we have copies of the documents you are looking for. It is the student’s responsibility to keep all copies of their documents in a safe place.

Not all forms are listed on this page due to regulatory restrictions. This includes DS-2019 extensions, work-authorization forms, reduced course load, transfer out, and more.
